This report includes a description of the fire event, a summary of the key findings and corrective actions as a result of the investigation. (Note: some corrective actions listed were identified during the process of the investigation but were not necessarily related to the incident).
A separate review of staff actions was also conducted. A worker who was underground during the emergency response and did not follow procedures has been dismissed.
